All visit pricing varies based on the client's insurance. For cash pay:

Chiropractic:
- New client initial appointments are around $250
- Follow-ups typically range from $55-$75
Functional Medicine:
- New client initial appointments are around $300
- Follow-ups typically range from $60-$115 (this does not include a chiropractic adjustment or any recommenced supplementation)
*Functional Medicine follow-ups are not billable to any insurance.
Primary Care:
- New client initial appointments typically range from $120-$200
- Follow-ups typically range from $105-$140
*All pricing depends on patients age and the purpose of the appt.

Please find the Client Forms section at the top of the menu on our website. Select New Patient Paperwork. Once completed, it will email a copy to our office. If you cannot fill it out online, please arrive 20 minutes prior to your scheduled appointment time to fill it out in office.
A new client initial appointment lasts up to 45 minutes and includes a consultation, exam, and initial treatment.
Missed appointment fees are as follows:

Functional Medicine Visit = $60
Chiropractic Adjustment = $30

We are unable to accept walk-in appointments, even for established patients. To respect the time of our physicians and our scheduled patients, we require an appointment to be made. If there is a medical emergency, please call 911 or head to the emergency room.
Please write a check or fill in your credit card number (including the security code and expiration date) on the back of the invoice and mail it back to Crossroads.
