*Massage Therapy is a cash pay service. We will not bill any massages to insurance.
All visit pricing varies based on the client's insurance. For cash pay:

Chiropractic:
- New client initial appointments are around $250.
- Follow-ups typically range from $55-$75.
Functional Medicine:
- New client initial appointments are around $300.
- Follow-ups typically range from $60-$115 (this does not include a chiropractic adjustment or any recommenced supplementation).
*Functional Medicine follow-ups are not billable to any insurance.
Primary Care:
- New client initial appointments typically range from $120-$200.
- Follow-ups typically range from $105-$140.
*All pricing depends on patients age and the purpose of the appt.
*Cash pay patients receive a 30% time of service discount. This price is reflected at checkout.

Massage Therapy:
- 30 minute massage = $45
- 60 minute massage = $90
- 90 minute massage = $135
*All massage is CASH PAY only.
Please find the Client Forms section on the top menu of our website. Select which paperwork is applicable for your scheduled appointment. Once completed, a copy will be emailed to our office.

If you cannot fill it out online, please arrive 20 minutes prior to your scheduled appointment time to fill it out in office.
A new client initial appointment lasts up to 45 minutes and includes a consultation, exam, and initial treatment plan.
Missed appointment fees are as follows:

New Client Visit = $100
Functional Medicine Visit = $60
Chiropractic Adjustment = $30
Massage Therapy = $45-$90

*All No Show Fees must be paid before rescheduling at our clinic.
We are unable to accept walk-in appointments, even for established patients. To respect the time of our physicians and our scheduled patients, we require an appointment to be made. If there is a medical emergency, please call 911 or head to the emergency room.
Please write a check or fill in your credit card number (including the security code and expiration date) on the back of the invoice and mail it back to Crossroads.
